New Patients

If you have recently moved into our area and wish to register with a doctor, please come in to the surgery where a receptionist will gladly help you.

How to check you are within our catchment area

New Patients wishing to be registered must live within the practice boundary of postal code L14 and L36 or within a two mile radius from the surgery or at the discretion of the doctor.  To check if you are within the practice catchment area please click here and enter your postcode. 

How to register as a patient at Pilch Lane Surgery

You can come into the surgery and complete a New Patient Questionnaire. Patients with chronic illnesses and those taking regular medication will be invited for a New Patient Check and medication review by the Practice Nurse.

Once registered, you may request to receive services from a particular doctor or nurse and we will endeavour to provide this whenever possible. If it is not possible we will explain this and offer alternative arrangements.
Please keep us informed of any change of address or contact details to ensure that we keep your records up to date and can contact you when required.

Temporary Patients

If you are in need of medical assistance whilst on holiday or visiting relatives in our area you can be seen by a doctor or nurse at our surgery should the need arise.

You will need to complete a form with your name, home address, temporary address and doctors details. Also, please bring details of any medication you may be taking with you when you come to see the doctor.

Zero Tolerance 

This practice operates a zero tolerance policy. Any violent or abusive patients will be removed from the practice

 


list.
